My 5 month old puppy has had Diarrhea 3 times in 2 months!?

He starts off with no appetite for a few days and then throws up and soon after that comes the Diarrhea. He doesn’t get dehydrated or weak, he remains very active as usual.

In mid October we took him to the vet and they gave us medicine and put him on a 3 day diet with “Science Diet – I/D” he was fine after that.
(The vet also said if he gets low on appetite again we can give him unsalted white rice with unseasoned chicken breast)
Then about 2 weeks later he had diarrhea again and we tried the rice and chicken but it just made his poop yellow, so off we went to the vet. They gave him a deworming medicine incase that was the issue and the vet sent us off with Immodium (to stop the diarrhea). About 2 days later he was perfectly fine. Then i found out that “Milkbone” treats can cause diarrhea too so i eliminated it and started giving him “Canine Carry Outs – Chicken”.

2 days ago i noticed his appetite started going away again.
Then this morning he was eating his food and he threw it up and did diharrea.
Then again tonight he went twice!

I don’t know if to change his food (Iams) or what! I don’t know what to do and my vet isn’t really giving me the answers I need to help identify what the problem is. I need HELP asap!
Yes he has been dewormed a few times already and i don’t feed him milk at all (Other then what was in the Milkbone that i wasn’t aware of) but my vet and various people have told me that Science Diet is the best ranked food and then Iams is very good also. But thanx for the answers so far!
